It can be ordered from Ernie Ball or your local Music Man dealer.

The one on mine is connected to the circuit board. It doesn't look like the ones on the ernie ball site or others.

Older pre-amps had the jack seperate from the board. Jacks for the newer ones are connected to the board. Both are still available from EB. The pre-amp for the SUB is the same as for the 2EQ Sting Ray pre-amp and both have the same jack. It has a black plastic four-sided casing with one side being diagonal.
