What bass were you using...if you don't mind saying!?
A Fndr HM - it was back in the EARLY 90's at the Alrosa Villa in Columbus (the place in Columbus to play back then) - we were running around the stage like banshees - big hair, ripped jeans, cheap wirelesses, instruments strapped real low and my guitarist (6'4") did his best Steve Vai throw... not to be outdone I gave it a try (me=5'8"... IF I'm having a great hair day). The combo of a longer neck, even longer strap and shorter bass player saw my bass fly over my shoulder, strap detach and bass plant, neck first, into the floor.
Chuncked the headstock, seperated the neck from the body (screws stripped out) and crushed my ego. If that were all that happened that night I would have been real happy... but it got much worse.
I grabbed my other bass and got through the rest of the night... almost. Got to the last song (think it was Bombtrack by Rage) and I had a "speaker malfunction." I had an acoustic 4x10, 1x15 stack - the speakers actually sat in their own tubes and the tubes had a neon blue paint around them - looked real cool at the time (with the grill off - which it was) - anyway, in the middle of the song and 3 of the 4 10s fell out of their tubes - pulling out the wiring, 2 of the cones ripped upon impact. Walked off stage and the owner said that in his 15 years of hosting bands he had never seen a player have a night like that. Very Spinal Tap-ish.
5 weeks later after a trip to the luthier and $250, it was like nothing happened to the bass (the cabs were scrapped for newer Peaveys) - and I made $35 that night (which was good $ on the CMH scene back then)
