They could either put a quilt top or a flame top on it if you just order a regular model. If you pay the $500 upcharge, you will get to choose whether you get a Flame or a Quilt, and it will be of premium quality figuring.
If I have this wrong, someone please correct me.
All EBMM guitars are made in San Louis Obispo, California, except for the Axis EX, which is made for the Japanese market. I believe all the parts are still made in California, but shipped to Japan for final assembly. They will not say anything on the headstock except Ernie Ball Music Man. No Axis, and no EVH signature. These will also be assembled in the EVH signature configuration, with the pickup selector on the lower horn. The EXs I've seen have a pretty maple cap, but did not look like a bookmatched figured maple cap.
If it says Axis on the headstock, it's American made. All EBMM guitars come with cases. Well, I'm not sure about the Sub 1 guitars, but I know the Axis does.
