Actually found this on emergenza.net; they say that EVH left EBMM in '95, but if memory serves me correctly, EBMM made EVH models in the same format, but minus signature until the first part of 1996. Correct me if I'm wrong, one of you good EBMM gents.

In 1990, Edward Van Halen selected MusicMan over a list of corporate competitors to build the Edward Van Halen Signature model guitar. The guitars were an immediate hit and retailers were unable to keep them in stock due to overwhelming demand.
When Edward left Ernie Ball in 1995 to pursue a more lucrative endorsement with a bigger company, the MusicMan E V H signature guitars became instant collectors items. MusicMan has replaced the EVH model with the Axis. The Axis boasts the same features as the old EVH model - plus several refinements - and is also available in a non-tremolo version.
I love this guitar. I loved it so much I bought another '91 exactly like it. Same colr. You could not tell the difference between them, But the 2nd one did not sound anywhere close to the first one so I sold it. I stll have the first one (pictured) and I still love it....
I have a evh trans purple hardtail. but I noticed someone said that after Eddie left they started offering that option. It has the EVH Signiture and The SN# is 86048 I dont know what year it was made. But I bought it used in 1997 Can anyone inlighten me. Did they Make EVH hardtails,

I got the DOB from dan. he said it was sold to a store in cental IL. It all adds up.
1994 is what year it was made. And I Bought it in 1997 in chicago at Guitar center.
